In order to receive your rebate (yes, 100%!) you must first buy the NeilMed NasoGel Spray from Walmart between the dates of March 16th and March 23rd. You must then complete the online form (found HERE), print it out and mail along with the original UPC and a copy of your receipt to the address listed near the bottom of the form. Your request must by submitted by April 15, 2011.
